Transportation Waiver
I give the transportation committee of Higher Ground Bus Company permission to transport my child from school to a general location near his/her home.I give permission for my child to walk and/or participate in activities geared for my childbut away from above mentioned Higher Ground Program locations under the supervisionof the authorized employees/volunteers of the Higher Ground Program.I further agree to assume responsibility for any liability which will arise out of ordinarynegligence otherwise with the exception of acts of gross negligence or intentional, willfulor wanton misconduct. On behalf of myself, my heirs, assigns, next of kin and child I waive for damages, injuries, etc… sustained to my child or property that I may have against the Higher Ground Program.
